• Receive care through secure electronic communication

• Acknowledge telehealth may not be a complete substitute for in-person care

• Withdraw consent at any point without affecting future care

All services are provided by licensed professionals in compliance with HIPAA and state regulations. Referrals for in-person care may be made when clinically appropriate.
