MEREWELL HEALTH
Financial & Appointment Policy
Effective Date: [10/18/26]
Merewell Health is committed to providing accessible, transparent, and convenient virtual healthcare. This policy explains our payment practices, insurance coverage, and appointment expectations.
1. Payment & Insurance
Merewell Health accepts eligible commercial insurance plans and self-pay patients.
Insurance acceptance does not guarantee coverage. Patients are responsible for verifying their benefits and understanding applicable copayments, deductibles, coinsurance, and noncovered services.
Payment for self-pay services and applicable patient balances is due at the time of service unless otherwise arranged.
When appropriate, Merewell Health may provide a superbill upon request for potential out-of-network reimbursement. Reimbursement is not guaranteed.
Eligible HSA and FSA funds may be used for qualifying services.
2. Government Healthcare Programs
Merewell Health does not currently participate in Medicare, Medicaid, TRICARE, or VA Community Care.
Patients covered by government healthcare programs should disclose their coverage before scheduling so Merewell Health can determine whether services can legally be provided and billed.
Important: Certain federal and state programs restrict private-pay arrangements. Merewell Health will not charge patients in violation of applicable program requirements.
3. Card-on-File Authorization
By agreeing to this policy, I authorize Merewell Health to securely maintain my payment method through its payment processor and charge it for:
Services rendered and disclosed patient payment obligations.
Applicable copayments, deductibles, and coinsurance.
Authorized cancellation or missed-appointment fees.
Other charges disclosed and agreed to in advance.
I understand that I may contact Merewell Health regarding billing questions or disputes.
Payment information is handled through secure payment-processing systems.
4. Appointment Cancellations & Rescheduling
Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment time.
Cancellations made with less than 24 hours' notice or missed appointments may result in a $30 fee.
Cancellation fees are not billed to insurance and are the patient's responsibility, where permitted by law and applicable payer agreements.
Merewell Health may waive a fee in cases of emergencies or other circumstances at its discretion.
5. Late Arrivals
Patients arriving more than 10 minutes late may need to reschedule their appointment.
Rescheduling is based on provider availability and clinical appropriateness.
6. Appointment Requirements
Required intake forms must be completed before the scheduled appointment.
Failure to complete necessary forms may result in an appointment being delayed or rescheduled.
Repeated missed appointments or late cancellations may result in limitations on future scheduling or termination of the patient-provider relationship, subject to applicable continuity-of-care and patient-notification requirements.
7. Patient Acknowledgment
By agreeing to this policy through Merewell Health's patient intake process, I acknowledge that:
I understand my financial responsibilities.
I understand the 24-hour cancellation requirement and potential $30 fee.
I authorize applicable charges to my payment method as described in this policy.
I agree to comply with Merewell Health's appointment procedures.
